Patient Portal In Development

Over the past few years, Primary Care Health Partners has been implementing an electronic health record system at all of our health centers.  Included in the next stage of this implementation is the roll out of a patient portal which is expected to enable you to perform such actions as:  secured messaging, prescription refill requests, appointment requests, and demographic updates.  We appreciate your patience.  Please check back for updates.

Some patients have noted we have had the message above on our website for over one year.  We regret our patient portal solution is not yet functioning but please know we are working on this.   At one of your upcoming appointments, you should see notices in the medical office when the portal is ready for use.  We appreciate your patience.
