Billing & Insurance

You should understand the coverage provided by your insurance plan. There may be limits as to which practitioners you can see or the services you can receive. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. You may also be responsible for amounts your insurance carrier considers non-covered services. If you have questions regarding your coverage, please contact your insurance carrier. Customer service phone numbers are usually printed on the insurance card.

PCHP participates with most local and many national insurance plans. You should note, however, that participation with insurance plans may be different for each office. Please refer to the table below to see if we participate with the insurance company providing your coverage at the location you will be visiting. Please also note, although we intend to keep this table current, it is subject to change without notice.
